End of Life Care

What Is End Of Life Supportive Care?

End-of-Life Supportive Care is a deeply compassionate, non-medical service focused on maximizing the comfort, dignity, and peace of mind for individuals facing a terminal illness. When your loved one chooses to spend their final chapter at home, our dedicated caregivers provide gentle, personal support that allows them to remain in familiar, comforting surroundings. We are here to support the physical and emotional well-being of the client, while providing essential relief and guidance to the family during this sensitive time.

We are not a substitute for hospice care, but we work seamlessly with your chosen hospice agency, providing the hands-on care and assistance that frees family members to focus on emotional connection and saying goodbye.

Those that benefit from our in-home end of life supportive care services

This service is for any senior with a terminal diagnosis who has chosen to remain in the comfort of their home, as well as the family members supporting them. It is for those who need:

  • Continuous Comfort: Individuals whose physical needs require constant, gentle attention to ensure pain and discomfort are minimized.
  • Emotional Presence: Clients who would benefit from a calm, supportive, and non-judgmental companion during their final days.
  • Family Relief (Respite): Family caregivers who are physically exhausted or emotionally overwhelmed and need time to rest, attend to personal matters, or simply be present with their loved one without the burden of constant physical care.

Our personalized in-home end of life supportive care services

Our goal is to ensure every day is spent with maximum comfort and dignity. Our compassionate caregivers provide support for the client and the family:

For the Client:

  • Gentle Personal Care: Assistance with all hygiene and dressing needs to maintain dignity and comfort (see ADLs below).
  • Comfort Measures: Repositioning and turning to prevent skin irritation and ensure ease of breathing.
  • Feeding and Hydration: Offering and assisting with nourishment as desired.
  • Meaningful Companionship: Sitting bedside, reading, talking, or simply providing a comforting, quiet presence.

For the Family:

  • Respite Care: Providing scheduled or emergency relief to allow family caregivers to step away and recharge.
  • Light Housekeeping: Managing essential home tasks to maintain a clean and peaceful environment.
  • Running Errands: Handling necessary duties like grocery shopping, picking up prescriptions, or fetching supplies.
  • Emotional Support: Offering an empathetic ear and guidance on working through this challenging period.

What are activities of daily living (ADLs)?

Activities of Daily Living (ADLs) are the essential daily tasks we perform for self-care, such as bathing, dressing, toileting, and transferring (moving).

During end-of-life care, the ability to perform these tasks declines, and it is vital that assistance is provided with the utmost tenderness and respect. Our caregivers are specially trained to assist with ADLs in a way that preserves the client's sense of self and prevents discomfort, making the difference between a difficult transition and a peaceful one.

A united approach

We believe that no one should face this journey alone. We commit to working as an extension of your family and in close collaboration with your hospice team, physicians, and other healthcare providers. By coordinating our non-medical support—like ADL assistance and companionship—with their medical expertise, we ensure your loved one receives a comprehensive circle of care, allowing their final days to be filled with love, connection, and peace.
